Sexual Sadism

Sexual sadism is a psychiatric aberration that usually starts in males in their late teens and twenties, and sometimes persists long term. The cause is unknown, but sometimes it is blamed on sexual assaults in childhood. Rarely, a brain tumour or disease may be found to be responsible for a personality change, and is detected by a CT scan.

The man has prolonged, recurrent and intense sexual urges and fantasies involving real or imagined acts in which physical or psychological suffering occurs in a victim. Humiliation is one of the more common psychological fantasies or acts. Most patients are unable to achieve sexual satisfaction in other ways. The victim may be consenting or non-consenting, and may be raped.

Psychotherapy and medications may be tried as treatments, but are often ineffective. Castration has been used as a last resort. Because of their interaction with the criminal justice system, many patients end up as long-term prisoners.
